Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. On outside threaded faucet with added splitter, vacuum breaker needed on both sides of the splitter.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Rice in reach in cooler at 47F. Per cook, rice was left out for approximately ten minutes. Rice moved to reach in freezer and approximately 15 minutes later temperature is 37F.

Per manager, two employees are missing proof of food handler training.

Establishment holds empanadas, croquetas, cheese/fruit pastries and butter on time controls. Establishment has paperwork with only establishment information at the top, no products listed or how items were to be marked and no signature. Inspector assisted in completing paperwork and also sent a blank form via email to the manager.
Label on a food item prepared and packaged onsite for customer self-service does not include product identity/description, date product was packaged and name and address of the establishment that prepared and packaged the food. Packaged cookies at the front counter for customer self service have no labeling information.
Wet wiping cloth not stored in sanitizing solution between uses. Wet wiping cloth on prep table. Manager removed.
