Employee washed hands in a sink other than an approved handwash sink. Observed 1 employee rinse hands in prep sink next to hand wash sink on cook line.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed mold like substance build up on soda gun nozzle behind bar. Person in charge stated soda gun was not in use and removed nozzle.
No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels for hand wash sink on cook line or near three compartment sink.
No tag on/for original container of raw, in-shell clams/mussels/oysters. Observed establishment selling raw oysters to the public but no tags are kept. No oysters on site during inspection.

Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. Observed raw shrimp in pan in standing water in three compartment sink. Employee stated shrimp was pulled to thaw 30 minutes prior. Person in charge moved shrimp to reach in cooler.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed; interior of smoker in back of establishment with food debris and grease build up. Microwave on wire rack near rear entrance soiled.
No tag on/for original container of raw, in-shell clams/mussels/oysters. See stop sale. Observed five (5) bags of raw oysters with no tag. Person in charge stated they remove tags and discard.
Spray bottle containing toxic substance not labeled. Observed bottle of clear chemical person in charge stated was sanitizer stored on prep shelf with food items near office.
Ceiling tile missing. Observed ceiling tile missing above office door.
Cutting board has cut marks and is no longer cleanable. Observed cutting board stored under prep shelf near rear entrance with grooves and no longer easily cleanable.
Dead roaches on premises. Observed four dead roaches in roach trap located next to fryer station across from smoker person in charge stated was not in use. Person in charge cleaned and discarded.
Floor soiled/has accumulation of debris. Observed; soiled floors under reach in cooler at back area of kitchen. Soiled floors under reach in cooler with bottled beers in bar area.
Food stored on floor. Observed container of raw oysters stored on floor while employee was shucking oysters. Person in charge moved container to proper storage levels.

Dead roaches on premises. In women's restroom, first stall had dead roach. Operator discarded.
