Dented/rusted cans present. See stop sale. Observed 4 cans of corn with dent along seam. One can of beans with dent along seam. Person in charge discarded.
Food-contact surfaces sanitized with a quaternary ammonium solution not exposed to the sanitizer solution for at least 30 seconds. Do not use equipment/utensils not properly sanitized. Observed employee wash rinse and briefly wipe sheet tray with quatanery ammonium, not submerging in quatanery ammonium for 30 seconds. Employee began filling sink with sanitizer to rewash and sanitize.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In three door reach in cooler cheese, cheese, ham at 46-49F. Ambient temperature of unit is 50F during inspection. Per person in charge were purchased and put in unit less than 4 hours ago. Moved time/temperature control for safety items to reach in freezer to rapidly chill.
No paper towels or mechanical hand drying device provided at handwash sink. No paper towels at hand wash sink in kitchen. Person in charge provided.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed debris buildup in pipe under left side of triple sink.
Outer openings not protected with self-closing doors. Self closer disconnected at back door.
Water leaking from pipe and/or faucet/handle. When in use, observed water leaking from pipe under far left side of triple sink. Pipe disconnected under sink. Person in charge connected during inspection and water no longer leaking.

Cutting board has cut marks and is no longer cleanable. Brown cutting board has deep groove marks. Person in charge states has on order. Threw away during inspection.
Standing water or very slow draining water in handwash sink, three-compartment sink, or mop sink. Hand wash sink in back kitchen slow draining due to stopper.

Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. At triple sink, quatanery ammonium set up at 0ppm. Person in charge added more quatanery ammonium and remade to 150ppm.
Ready-to-eat, time/temperature control for safety food marked with a date that exceeds 7 days after opening/preparation. Turkey date marked out more than 7 days. Opened 3/28 marked for 4/4. Person in charge marked appropriately.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Establishment utilizes quatanery ammonium to sanitize, unable to provide quatanery test strips.
Water with a temperature of at least 100 degrees Fahrenheit not provided/shut off at employee handwash sink. Hand wash sink in bathroom with water at 77F.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling vents in kitchen with dust buildup.
Equipment in poor repair. Observed left side double door reach center interior support in poor repair repaired with tape.
