Person in charge stated cooler was left open for order and for cleaning. Person in charge had items moved to another cooler without date marks lower ambient temperature.
Warewashing sanitizing solution exceeding the maximum concentration allowed. Observed Sanitizer Bucket x 2 (Quaternary 500++ppm); person in charge remade buckets to Sanitizer Bucket (Quaternary 200ppm)

Operator placed all items in ice bath. In reach-in cooler in back of building next to triple sink, black beans (47F - Cold Holding). All re temps at or below 41F, except for salsa at 48F. Operator placed salsa in different cooler.
Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. In reach-in cooler across from triple sink, Baja sauce (46F - Cooling, 2nd temp after 2.5 hours 45F); in large plastic covered container cut lettuce (58F - Cooling, 2nd temp after 2.5 hours 48F) in multiple bags stacked on each other. Per operator, Baja sauce and cut lettuce had been prepped 1 hour prior. Operator placed all times in different reach-in cooler.
