Incorrect chemical test kit provided for measuring the concentration of the sanitizer solution used in the three-compartment sink and/or dishmachine. Operator provided pH test kit, utilizes quaternary sanitizer.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Provided operator employee reporting agreement, (HR 5030-309) manager health reporting responsibilities (HR 5030-101)and employee health reporting responsibilities (HR 5030-103], discussed with operator importance of employee heath. Discussed employee heath with operator, operator filled out forms with all employees at time of inspection.
Water with a temperature of at least 100 degrees Fahrenheit not provided/shut off at any sink. Operator states hot water heater is broken, and has purchased replacement. Temperature 74F.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Cook has jewelry on wrists.

Opened employee beverage container in a cold holding unit with food to be served to customers. Observed opened drink in tall white reach-in cooler for raw chicken. Operator removed the beverage.
