Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed cook drop spoon and proceed to pick up and placed the three compartment sink. Then the cook proceeded to grab a clean plate. Had operator to stop employee. Operator explained that hands needed to be washed after handling the dirty dish. The cook washed and dried hands.
Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw beef over sour cream on the shelf near the walk in cooler door. Operator placed the raw beef under the container of sour cream in side the walk in cooler.
No paper towels or mechanical hand drying device provided at handwash sink in the bathroom.
Buildup of food debris/soil residue on equipment door handles near the fryer and at the pizza make station.
