Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Inside top of refrigerated make-table: cooked onions (64F - Cold Holding). The onions are sitting on top of other pans inside the make-table, above the safe load limit indicated inside the cooler. Manager stated the onions were placed here from another cooler one hour prior, then moved them into the bottom of the cooler to cool to 41F.
Raw/undercooked animal food offered and establishment has no written consumer advisory. Raw animal foods must be fully cooked prior to service. No consumer advisory on menu or posted anywhere in the restaurant for undercooked eggs offered on the menu. Manager printed and posted the consumer advisory during this inspection.
Carbon dioxide/helium tanks not adequately secured. One co2 tank not secured beside three-compartment sink.
Food storage container/container lid cracked or broken. One cracked clear plastic pan on clean dish rack. Manager discarded the pan during this inspection.

Wet mop not stored in a manner to allow the mop to dry. Mop stored on floor near three-compartment sink. Manager hung the mop to dry at the mop sunk during this inspection.
