Handwash sink not accessible for employee use at all times. At the sushi station, the sink has strainer bowls stored inside. The sink was cleared during the inspection.

Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. -Observed no time mark on sushi rice held using time as a public health control. Per operator, rice made 1.5 hours prior to the inspection. Operator correctly time marked rice.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed accumulation of black mold like substance on can opener blade.
No chlorine chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths.
Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed no date marked on sauce stored in walk in cooler. Per operator, sauce made yesterday morning.
Bowl or other container with no handle used to dispense food. Observed no handle bowl used to dispense sugar.
Food stored on floor. Observed container with ginger stored on floor at wait station. Operator removed and stored properly.
Storage of maintenance equipment/tools in areas that may result in cross contamination of food, equipment, utensils, linens and/or single-service and single-use articles. Observed drill set stored on food storage shelf over containers with rice. Operator removed and stored properly.
Utensils in poor condition. Observed spatula with deep chipped. Operator discarded at will.
