Server came into kitchen and scratched back of head with bare hands then continued to handle single use items and basket of food of customers without washing hands after touching hair.
Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Triple sink at bar reading 0ppm, manager added more sanitizer tablets.
Toxic substance/chemical improperly stored. Spray bottle of bathroom cleaner stored with napkins in storage room next to bar.

Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. Food stored on floor. In the storage room to the side of the bar counter, jugs of water stored on the floor.
In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. In the waitstation area, serving spoon for queso stored on the side of the hot holding container on a paper towel with leftover queso on the spoon. Operator cleaned and relocated the spoon to the container of queso.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Walk in cooler door gasket has build up of mold like substance spots. Sliding door reach in freezer has food debris in the tracks of the sliding door.
Stored food not covered. On the sliding door reach in freezer, container of veggie burgers not stored covered. Operator covered the veggie burgers.
Working containers of food removed from original container not identified by common name. In the dry storage room in the bottom shelf, bulk flour container with no product labeling. Operator labeled the bulk container.
