Observed brown rice (84F - Hot Holding) located at rice warmer and "Poke Mia" food station area, due to warmer not turned on and not plugged into outlet, less than 4 hours. Employee plugged in kitchen equipment and turned on rice warmer to reheat brown rice to 135F and above.
Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed diced raw tuna in commercially processed reduced oxygen packaging, not removed from packaging prior to thawing, stored inside reach in cooler. Discussed with employee proper thawing requirements, employee began to remove fish from ROP packaging.
Single-service articles stored below soap or paper towel dispenser exposing the items to drips from people's hands. Observed a container of plastic food wrap stored under paper towel dispenser at "Poke Mia" food station. Employee removed plastic food wrap from under paper towel dispenser.

Observed food employee washing utensil at prep sink located at Poke Mia kitchen unit, without sanitizing, then proceeded to dry utensils with wiping cloth before storing. Discussed with Manager proper wash, rinse and sanitize procedures.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed shrimp (46F-47F - Cold Holding) located at reach in cooler drawers, due to the constant opening and closing of cooler drawers, ambient temperature on cooler observed at 37F. Advised to Manager to submerge food containers in ice to maintain +or- 41F and below.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed white rice (90F - Hot Holding); brown rice (114F - Hot Holding), both items stored in rice warmers located at Poke Mia kitchen unit. As per Manager items were prepared at 11:30am, less than 4 hours. Advised to reheat to 135F and above.
Clean equipment/dishware/utensils stored below soap or paper towel dispenser exposing the items to drips from people's hands. Observed employee dicing onions on a cutting board, stored directly under paper towel dispenser at Poke Mia kitchen unit.
Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner located above prep cooler at Poke Mia kitchen unit.
Cutting board has cut marks and is no longer cleanable located at prep cooler at Poke Mia kitchen unit.
Employee with no hair restraint while engaging in food preparation. Observed food employee engaged in preparing a poke bowl at front food prep cooler, with no hair restraint. Manager provided a hair net to employee, at time of inspection.
Food not stored at least 6 inches off of the floor. Observed plastic containers of soy sauce stored on floor at dry storage area.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed utensils/rice spoons stored in standing water at 75F located next to rice warmers. Manager discarded standing water.
In-use wet wiping cloth/towel used under cutting board located under a green cutting board at shared kitchen area.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed soiled microwave located next to exit door leading to dry storage area.
