Observed multiple employees food safety training expired in 2025. Also, observed Shania and Kenia have a food safety training certificate from an unapproved vendor. The certificate only has servsafe on food safety training certificate.
Objectionable odors in bathroom or other areas of the establishment. Observed an objectionable odor in hall area near restrooms.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. raw salmon, raw beef (50F-51F - Cold Holding) in reach-in cooler drawer below grill. Operator had employee place containers of ice next to products to chill items.
Toxic substance/chemical improperly stored. Observed spray bottle containing toxic substance stored next to water pitcher and condiments. Operator removed spray bottle of cleaner to properly store.
Food-contact surface soiled with food debris, mold-like substance or slime. Cutting board on reach-in cooler on cook line next to grill soiled/ stained. Also, cutting board on reach-in cooler on front service line soiled/ stained. Also, ice deflector plate in large ice machine soiled with mold like substance. Also, underside of large mixer in prep area soiled with accumulation of dried food debris.
Required employee training expired for some employees. Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior of large ice bin soiled with mold like substance.
Equipment and utensils not properly air-dried - wet nesting. Clean stacked pans on storage shelf in prep area unable to properly air dry. Operator started to separate pans that are stacked.
Equipment in poor repair. Ice deflector plate in large ice machine chipped-missing a piece of the deflector plate.
Floor soiled/has accumulation of debris. Observed floor in walk in freezer soiled with accumulation of debris. Also, floor around fryer equipment soiled with accumulation of food debris and grease.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed Gasket on reach-in cooler drawers near egg station soiled with food debris. Also, multiple fans on cook line, front service line and prep area soiled with accumulation of dust and debris. Also, bump bar on cook line to remove orders soiled with accumulation of debris.

Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed chemical dish machine at 0ppm, noticed the chemical sanitizer jug was empty. Manager was able to get Dish machine to operate properly. Took test of dish machine at 100ppm.
Employee cracked raw shell eggs and then handled ready-to-eat food and/or clean equipment or utensils without washing hands. Observed employee crack raw shell egg and proceeded to handle equipment and utensils. Manager intervened and coached employee hand washing. Employee removed gloves and washed hands.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed in reach in cooler in front of kitchen Sour cream (52F - Cold Holding); Shredded cheese (53F - Cold Holding); Cut diced tomatoes (53F - Cold Holding); House made salsa (56F - Cold Holding). Observed gaps in drawers of reach in cooler. Cooler has an ambient temperature of 46F. Manager removed TCS items from cooler and placed in another cooler.

Ambient temperature of cooler at 44F. Manager stated that cooler door was opened for a period of time for food order delivery. Opened freezer door until cooler is looked at.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed mop sink with splitter installed without back flow device.
