Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In reach in cooler: ground beef 54F. Beef was out of refrigeration during rush.
Wiping cloth solution exceeding the maximum concentration allowed stored in a location that could result in the cross contamination of food, equipment, utensils, linens, single-service, or single-use articles. Chlorine sanitizer 200+PPM. Employee added water then 75PPM.

Mobile food dispensing vehicle license number not permanently affixed on the side of the unit in figures at least 2 inches high and in contrasting colors from the background of the vehicle.
Raw fruits/vegetables not washed prior to preparation. Pan of avocados with stickers in place in top of reach in make table. One avocado in pan has been sliced.
