In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. In the bin of flour, the scoop was buried in the flour. The scoop was moved to stand up.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. In the prep area, a bucket of 120F water was holding knives. The water was removed.

Toxic substance/chemical improperly stored. On the dry storage rack next to the office, two spray bottles of cleaner hanging on the shelf with croutons and chicken base and over cookies and sugar. The bottle as were relocated.

Paper towel dispenser at handwash sink not working/unable to dispense paper towels. At the cook line hand wash sink. The batteries were dead. The batteries were replaced.
