Live, small flying insects found. Observed two flying insects in unit during inspection with service window and door open.
Shell eggs not stored at an ambient air temperature of 45 degrees Fahrenheit or less. 6 dozen shell eggs stored in ambient storage cabinet under cook line at 79F ambient. Per person in charge has been there for 1 hour. Moved to reach in cooler holding at 41F ambient.
Stop Sale issued on time/temperature control for safety food due to temperature abuse. Per person in charge dal made with cooked lentils last night at 46F in reach in cooler. All other items in unit in temperature.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Chutney with cooked tomato stored above fill line in pan in reach in cooler at 48F. Person in charge moved to reach in cooler below.

Ceiling tile missing. Vent and light cover missing on ceiling of unit. Also, floors under cabinets at cook line and triple sink with debris accumulation.
