Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. diced tomatoes (56F - Cold Holding at 2:20, placed in cooler at 12:00, retemp 48F at 3:05); lettuce (54F - Cold Holding at 2:20, placed in cooler at 12:00, retemp 48F at 3:05 ); black bean corn mixture (54F - Cold Holding at 2:20, placed in cooler at 12:00, retemp 47F at 3:05) Cooler was not holding temp. Manager placed items in freezer to help bring temperature down quickly.
Ceiling tile missing in front of bar counter and behind bar counter. Spoke with operator; they are missing due to a roof leak that happens when it rains. Operator stated they are in contact with a company to come fix the roof but that they haven't received a date or time they'd be coming out. Operator closes those areas off when leaking does occur and doesn't keep and food or food contact items in those areas for the time being.

Ceiling tile missing. 3 ceiling tiles missing near and at bar area due to active water leak from roof damage. Water is not leaking onto any food or food contact surfaces as they've moved the seats from that area of the bar. New ownership has called a company to come fix the roof.
