Toxic substance/chemical improperly stored. Spray bottle of super cleaner stored on top of box with paper towels rolls, manager moved the cleaner.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Splitter added, no back flow preventer installed at the non chemical side of the splitter.
Handwash sink not accessible for employee use at all times. Handwash sink at the cook line, container with scrubbing pads inside sink, manager removed container.
Single-service articles improperly stored. At the drive thru area, single service (fries containers) not covered/inverted, manager covered.

Male employee pushed glasses onto face then continued to touch sandwiches without washing hands. Male employee touched apron and continued to hand customer drink cup without washing hands. Male employee touched apron then continued to pick up sandwich without washing hands.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Beef on slicer 112F on top and 121F. Manager stated beef is on slicer no longer than thirty minutes. Manager voluntarily discarded.
Food-contact surface soiled with food debris, mold-like substance or slime. Soda chute at drive thru has build up of slimy substance. Interior of reach in freezer near fryers soiled with food debris.
Handwash sink not accessible for employee use at all times. Sink on cookline filling with water and not draining, resulting in employees not being able to utilize the sink for hand washing.

Employee with ineffective hair restraint while engaging in food preparation. Female employee wearing hat has long pieces of hair over face. Manager addressed with employee and hair was pulled back.
