Handwash sink not accessible for employee use at all times. Sheet pan rack of bread was placed in front of the hand washing sink that is located across the reach in cooler. Operator removed rack for easy access to hand washing sink.

Accumulation of black/green mold-like substance in the interior of the ice release chutes of soda dispensers. Operator had areas, cleaned and sanitized while inspection continued.
Floor soiled/has accumulation of debris. In dining room, observed debris under two waste receptacle stations. Operator had employee clean area with a broom and dust pan.
Floor tiles missing and/or in disrepair. At Three compartment sink area observed tile in disrepair around floor drain.
Single-service articles not stored inverted or protected from contamination. Observed single use cups and lids not inverted. Operator inverted cups after we discussed potential for contamination with airborne particles.
