Employee at front counter pushed glasses up on her nose and immediately handled clean tongs working with food items without washing hands. Manager coached employee on proper hand washing procedures. Employee washed hands correctly.

Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control has been greater than four (4) hours. See stop sale. On cook line in kitchen, container of fried onions prepared more than six (6) hours prior does not have time mark. Fried onions discarded.

Paper towel dispenser at handwash sink not working/unable to dispense paper towels. Paper towels dispenser in women's restroom unable to dispense. Operator placed new batteries in dispenser but still unable to dispense. Operator removed paper towels from dispenser to make available.

Food stored on floor. Observed 2 cases of frozen fries stored on freezer floor. Manager had employee complete the stocking and remove cases of fries from freezer floor.

Water leaking from pipe and/or faucet/handle. Observed water leaking on to floor at hand sink located in the middle of kitchen. Employee was able to align pipe so that water does not leak on floor.
