Floor area(s) covered with standing water. Standing water on floor in front of walk in cooler door. Per operator leak from hose connection normally caught by mop bucket. Operator placed mop bucket under hose.
Ice buildup in reach-in freezer and/or walk-in freezer. Ice buildup up on top portion of stand up reach in freezer under compressor.

Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Sanitizer solution next to three compartment sink at 0ppm, operator emptied and replaced with solution at 400ppm.

Toxic substance/chemical improperly stored. At dry storage area, case of restroom cleaner stored on top shelf above single use gloves. Operator moved chemical to correct storage.
