Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Fries Station-; Cooked Fries (118-120F - Hot Holding) Per operator food items in unit less than 2 hours. Operator discarded.
Employee applied hand antiseptic in place of washing hands as required. Employee began work while using personal phone proceeded to use hand sanitizer instead of washing hands while continuing working with food. Reviewed proper handwashing procedures. Employee washed hands correctly.
No paper towels or mechanical hand drying device provided at handwash sink front of Walk in Cooler. Replaced during inspection.
Bottom shelf of Traulsen Freezer soiled with food debris.

Ceiling/ceiling tile shows damage or is in disrepair. In the storage closet towards back door, there is a large hole surrounding pipe.
Floor soiled/has accumulation of debris. Floors soiled with paper debris and trash under condiment storage and front counter. Operater started cleaning.

Warewashing sanitizing solution exceeding the maximum concentration allowed. Ware washing solution was quaternary 400ppm. Advised operator to correct to quaternary 200ppm.
Reach-in cooler interior/shelves have accumulation of soil residues. Inside reach in freezer soiled with food debris.

Raw animal food stored over/not properly separated from ready-to-eat food. Observed in low boy cooler at make table, raw burgers stored next to cut tomatoes. PIC removed and stored appropriately.

Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. Observed cut tomatoes (47F @ 12:15- Cooling since 9:00, second temperature 44-46F at 12:50). Items were placed in low boy to cool with lid on. Pan also had too much tomatoes cooling at once. PIC separated tomatoes and added ice to rapid cool.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed - Dr Pepper soda nozzle at drive through soiled with mold like debris. - Coke soda nozzle in dining area soiled with mold-like debris.
Working containers of food removed from original container not identified by common name. Observed brown sugar stored in metal pan in dry storage area not identified by it's common name.

Observed no proof of training for employees employed after 7/22/2021.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. Drive through Observe in-use utensil standing at room temperature at 89F. Educated manager of proper storage method for in use utensils. Manager discarded.
