Observed employee engaging in food preparation touch face with gloved hands and proceeded to engage in food preparation. Manager intervened and employee removed gloves and washed hands properly.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed on counter near reach in cooler Butter (63F - Cold Holding). Manager stated the pan was supposed to go inside the reach in cooler. Manager placed inside reach in cooler. Observed on inside ledge of reach in cooler Cream sauce (52F - Cold Holding) in soufflé cups. Manager stated he pulls out certain amount during rush. Manager placed inside reach in cooler.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed threaded hose Bibb located near mop sink without a back flow device.

Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Observed hot water faucet at employee hand washing station on cook line in disrepair, will not turn on.
Food stored on floor. Observed on cook line a box of cooking oil stored under employee hand washing sink. Employee removed and stored properly.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed walk in cooler door gasket soiled with mold like substance. Observed filters over grill area soiled with grease.
