Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet.
Employee has not received adequate training related to their assigned duties as evidenced by lack of general knowledge in performing their assigned duties. When asked what kind of sanitizer is being used, employees indicated they are using soap, no sanitizer. Provided education to both employees on site to wash, rinse and sanitize utensils being used.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Provided some quat strips. Operator placed order.
