No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at handwashing sink in bar area. Operator replaced paper towels.
No soap provided at handwash sink. Observed no hand soap at handwashing sink in bar area. Operator replaced soap.
Bowl or other container with no handle used to dispense food. Observed bowl being used as a scoop in sushi rice. Operator removed scoop.
Food stored on floor. Observed a case of diced potatoes on the floor in the walk-in freezer. Operator moved item to shelf.
