Rodent activity present as evidenced by rodent droppings found. Observed approximately 25 rodent droppings on the shelves where they store the alcohol in the back dry storage area.
Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee touch pants without changing gloves or washing hands on the cook line and resume food preparation, informed employee importance of hand washing and changing gloves.
Handwash sink not accessible for employee use at all times. Observed hand wash sink by 3-compartment sink was inaccessible by a pan with bottles in it, operator moved.
Handwash sink used for purposes other than handwashing. Observed hand wash sink by the ice machine with lettuce in it.

Manager redirected employee to wash hands after handling dirty dishes then return to work.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed during lunch rush preptop cooler (); sour cream (47F - Cold Holding); cut lettuce (47F - Cold Holding); shredded cheese (50F - Cold Holding) foods in cooler less than 1.5 hours. Operator will monitor foods and sell all by 430 pm or discard. Operator turned unit down to 34 F and will monitor unit to be sure foods remain at 41F or below or return to 41F or below within 4 hours.

Observed upright stainless one door cooler at end of makeline (); beef (46F - Cold Holding); fish (46F - Cold Holding); chicken (44F - Cold Holding). Unit not able to maintain foods at 41F and may not be used until able to do so. Operator states Food were loaded abou 45Min ago and put all back in walk-in cooler to quickly cool to 41F within 4 hours. Operator will not use unit until repaired.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed; cooked peppers (119F - Hot Holding) in pan held half on grill and half off grill. Operator placed fully on grill to bring to 165 then will hot hold at 135F or hotter.
Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Inspector provided handout.
