Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee on cell phone, put gloves on without washing hands, then touched clean utensils on cook line. Operator discussed with hand washing procedures with employee.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. cut cabbage, cut tomatoes, house made salsa, goat cheese, cooked egg (49F-53F - Cold Holding) in top of reach-in cooler across from glass door cooler on cook line. Operator stated equipment was just worked on yesterday. Operator had employee create an ice bath and placed it under TCS foods in top of reach-in cooler to chill. Operator stated items placed in cooler this morning. Rechecked temps at end of inspection and temp dropped a couple of degrees
Toxic substance/chemical improperly stored. Two spray bottles containing toxic substance store with nozzle facing single service to go containers on shelf in bar area. Operator removed spray bottles of cleaners/ sanitizer to properly store. Also, container of soap stored on shelf next to wrapped sleeves of single service cups in front service station. Operator removed container of soap.
Warewashing sanitizing solution exceeding the maximum concentration allowed. Triple Sink (Quaternary +500ppm) test strip turned blue exceeding maximum concentration allowed at triple sink in bar area. Operator stated she will call to have service.
Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Sanitizer Bucket (Quaternary +500ppm) on cook line. Test strip blue which exceeded maximum concentration allowed. Operator asked employee to remake solution.
Food-contact surface soiled with food debris, mold-like substance or slime. Multiple portable cutting boards on cook line, cutting board on reach-in cooler on front service line soiled/ stained. Also, ice chute in large ice machine on right soiled/ has accumulation of pink substance. Also, two soda guns in bar area soiled.
Handwash sink used for purposes other than handwashing. Employee used Hand wash sink near ice machine or fill pitcher of water. Operator discussed with employee.
No paper towels or mechanical hand drying device provided at handwash sink. No paper towels at hand wash sink in ware washing area and no paper towels in bar area. Operator replenished paper towels at both hand wash sinks.
Required employee training expired for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Multiple employees food safety training expired in Feb and June of 2025.
Buildup of food debris/soil residue on equipment door handles. Build up of debris on lids of multiple containers of seasoning on shelf in dry food storage area. Operator changed out lids.
Equipment and utensils not properly air-dried - wet nesting. Clean stacked pans on storage shelf in prep area unable to properly air dry
Ice buildup in reach-in freezer and/or walk-in freezer. Ice buildup up on floor and under shelf in walk in freezer.

Container of medicine improperly stored. Observed bottle of medication stored on top of cooler in bar area. Manager removed and stored properly.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed in reach in cooler Spinach dip (52F - Cold Holding) and Raw chicken (53F - Cold Holding) in drawer cooler. Employee stated spinach dip was brought out at 11am. Manager placed ice under spinach dip. Observed raw chicken stored next to open space in drawer cooler. Manager placed pan into cooler to help with air flow and placed bag of ice in raw chicken.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed hose Bibb next to Dish machine with out back flow device. Manager provided back flow device for hose Bibb.
Handwash sink used for purposes other than handwashing. Observed employee using employee hand sink to fill large pail for water. Manager educated employee regarding employee hand wash sink is used for only hand washing.
Bowl or other container with no handle used to dispense food. Observed small cup without handle used to portion sauce located in reach I cooler at entrance of kitchen. Manager removed and stored properly.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed multiple employee drinks on clean prep tables on cook line and expo area. Manager removed and stored properly.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed employee engaging in food preparation wearing watch.
Light shield damaged/in disrepair. Observed light shield missing in walk in cooler.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed walk in cooler door gasket soiled.
Time/temperature control for safety food thawed in an improper manner. Observed frozen salmon in large container of water. Manager turned water on salmon thawing.
Wiping cloth sanitizing solution stored on the floor. Observed wet wiping cloth solution stored on cook line floor. Manager removed and placed another bucket under sanitizer bucket.

Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Uncut Packaged tuna, with label requiring to be cut or removed, in walk in cooler.
Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Dish machine conveyor rack not moving. Work in progress. Service technician onsite
