Food-contact surface soiled with food debris, mold-like substance or slime. Observed soda guns and soda holsters in outside bar with mold like substance.
Handwash sink used for purposes other than handwashing. Observed sanitizer bucket in hand wash sink in grill kitchen. Manager removed.
Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed tuna in ROP packages with no way for air to escape. Manager put holes in all packages.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed speaker on cutting board, backpack on top of glasses and personal drink on prep table. Manager removed all items and had employees clean and sanitize the area.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Tested at 0ppm. Dip stick was not in product. Fixed and primed, retested at 50ppm.

Ceiling/ceiling tile shows damage or is in disrepair. Above three compartment sink
Cove molding at floor/wall juncture broken/missing. Under pizza hand wash sink, corners by walk-in cooler and cook line.
Exterior door has a gap at the threshold that opens to the outside. By pizza station, and at entrance near cook line.
