Observed that the pan was double panned. Operator removed 2nd pan so that egg is closer to heat to heat egg back up.
Food-contact surface soiled with food debris, mold-like substance or slime. Cutting board at reach-in cooler across from hot holding box soiled/ stained. Also, deflector plate in ice machine soiled.
Spray bottle containing toxic substance not labeled. Two Unlabeled Spray bottle containing toxic substance stored under front counter. Operator labeled spray bottles of cleaner/ sanitizer.

Wiping cloth chlorine sanitizing solution not at proper minimum strength. Sanitizer Bucket (Chlorine 0ppm) below hot holding table. Operator added bleach to bucket and retested at 100ppm
