Observed no proof of any food handler training for staff over 60 days hired. Manager is working on updating the training for all employees.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed AC vents above make line soiled with dust and debris.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed pork (45F - Cold Holding); beef tubes x 4 (45F - Cold Holding); house made slaw (46F - Cold Holding); pasta salad (44F - Cold Holding); cut lettuce (46F - Cold Holding) in walk-in cooler. Ambient walk-in cooler 45F. Person in charge called service. Walk-in cooler was 41 or below at 11 when establishment opened per person in charge. Person in charge place ice bags on all products in walk-in cooler.
Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking. Observed egg bath on ice missing time control label. Person in charge added time control label.

Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed fish not removed from its reduced oxygen environment while thawing, fish was also sitting in water. Person in charge drained fish and sliced open vacuum sealed bags and placed fish in walk-in cooler to continue to thaw.
