Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed dish machine chlorine sanitizer at 0ppm. Operator discontinued use of dishwasher machine for sanitizing and set up 3 compartment sink for sanitizing.
Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee start to put on gloves on without washing hands. Educated employee on handwashing and changing tasks. Employee removed gloves and washed hands then put on gloves.
Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Observed chlorine wiping cloth sanitizer bucket on front line, to strong. Operator diluted chlorine sanitizer. Chlorine sanitizer bucket now at 100ppm.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling vents in dish area with soil and dust.
Hole in or other damage to wall. Observed wall next to mop sink in back food preparation room with a hole approximately 12" x 6" just above the cove molding.
