Food-contact surfaces not sanitized after cleaning, before use. Do not use equipment/utensils not properly sanitized. Observed employee washing and rinsing dishware, no sanitizer step in back of kitchen. Spoke with employee, employee filled third sink with quaternary sanitizer and returned items to sink for proper sanitizing.

Counter warming unit at entrance: cooked rice (109F - Hot Holding) for 15 minutes per operator cooling. Operator states rice was removed from warmer during catering order. Operator replaced with new rice and returned rice to hot water bath on flat top for rapid reheating.
