Live, small flying insects found. Two flies flying in kitchen near three-compartment sink.
Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Pan containing raw hamburger meat stored on shelf above pan containing raw shrimp inside bottom of reach-in cooler at cookline. An employee moved the raw hamburger meat to the bottom shelf during this inspection.
Toxic substance/chemical improperly stored. Spray bottle containing bleach stored on prep table above bags of hamburger buns in prep area of kitchen. Manager removed the bleach from the table during this inspection.
Food-contact surface soiled with food debris, mold-like substance or slime. Bottom edge of deflector plate inside ice machine soiled with mold-like substance.

Handwash sink used for purposes other than handwashing. Ice dumped into handwash sink near cookline.
Cleaned and sanitized equipment or utensils not properly stored. Several boxes containing single-use containers stored directly on floor in office.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee phone stored on shelf beside microwave, above hamburger buns. An employee removed the phone from the area during this inspection.
In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Handle of scoop stored down inside sugar inside bin near cookline. Manager removed the scoop from the bin during this inspection.
Unnecessary items/unused equipment on the premises. Several unused coolers, fryers, and other equipment stored outside behind restaurant.
Wet wiping cloth not stored in sanitizing solution between uses. Wiping cloth stored on prep table in kitchen. Manager moved the cloth back to a sanitizer bucket during this inspection.
