Food not prepared or portioned today. -right side flip top- raw shell eggs (50F-Cold Holding); cut lettuce (50F - Cold Holding), observed lettuce stored on double pannel. Employee removed food out of temperature -NORLAKE-; shredded cheese (49F - Cold Holding); cheese cake (48F), observed unit in disrepair. -kitchen- prep table-butter (72F - Cold Holding), observed butter left to ambient temperature. Per employee food held less than 4 hours, operator moved all time temperature food to a proper temperature cold holding unit of 41F or below for a quick chill.
Handwash sink not accessible for employee use at all times. Gallon of water on middle of sink, and 3 boxes in piles in front of hand wash sink in cook line. Employee removed all.
Ceiling tile missing. 3 units above triple sink. Per employee tiles were moved to fix a leak.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. NORLAKE- broken bottom gasket - ambient thermometer at 50F. Advised operator not to stored any time temperature control food on unit, until technician verifies proper cold holding temperature of 41F or below.
