Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Several time/temperature control items over 41F in sandwich maketable: sliced tomatoes 50F, diced tomatoes 47F, sliced turkey 44F, shredded lettuce 48F, shredded cheese 48F. Per operator, items in maketable for at least 3 hours. Items were moved to walk in freezer at the time of inspection for rapid cooling.
Toxic substance/chemical improperly stored. Spray bottle with sanitizer and two sanitizer buckets over ketchup bottles and mayo packets, and next to paper napkins in rack at the server station.
Accumulation of black/green mold-like substance in the interior of the ice machine.
Holes/cracks in ceiling tiles in the following locations: over the dish machine in the ware washing area, over handwashing sink next to hood vent in the cook line.
