Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed employee touching a hamburger bun with bare hand contact during food preparation, employee discarded hamburger bun, washed hands and put gloves on.
Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw steak over a pan of diced green peppers and a pan of diced onions, operator moved steak to the bottom shelf.
Cutting board has cut marks and is no longer cleanable. Observed cutting board on the cook line grooved.

No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed expired test strips, owner showed they have been ordered.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed white guard in the ice machine with a black like substance on it.
Bowl or other container with no handle used to dispense food. Observed soufflé cups in the Cole slaw and salsa, owner removed.

Observed foods in steam table corn (131F - Hot Holding); tomato sauce (128F - Hot Holding); cheese sauce (113F - Hot Holding); water on steam table at 131F operator stated foods restocked at less than 1.5 hours ago. All were reheated in microwave to 165F and steam table heat was increased to hold reheated foods at 135F or hotter.
Employee with long fingernails working with exposed food, single-service items or clean equipment and utensils. Observed waitstaff engaged in serving open foods, handling clean equipment and single service items, dipping ice, with long nails.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Obse4ved cook on line with wrist band/ bracelet. Observed waitstaff engaged in handling open foods taking plates to tables and dipping ice with filigree / cut metal rings.

Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed ROP fish labeled remove from package and thaw under refrigeration, sitting on counter thawing at room temperature in completely sealed packages. Product partially frozen. Operator removed from packaging and put in walk-in cooler to continue to thaw.

Observed 9 live roaches under shelving in same room where unpackaged single service items are stored. Observed 7 live roaches under 3 bay sink at warewash area. Floor soiled/has accumulation of debris. Observed old food debris buildup behind equipment and on shelving at breading station. On floor under cookline and at corners of floor heavy blackened debris and old food debris observed
Grease on the floor around grease receptacle located inside establishment at back door. Observed grease overflow empties on to floor.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Legs and exterior of prep tables at cookline have old blackened food debris on surfaces. Observed black debris accumulated on top of filter housing at drink station area.
Single-service articles not stored inverted or protected from contamination. Observed water damaged area of stack of take out boxes in dry storage area. Manager discarded affected items.
Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Found at 0ppm after approximately 30 minutes standing. Observed Quaternary ammonia dispenser attached and dispensed through hot water tap. Found at 108F. Directions on quaternary dispenser suggest 68F to 75F water be used. Hot water causes the sanitizer to dissipate into the air leaving the solution.
