Person in charge unable to provide proof of training for employee during inspection.
Nonself-sufficient mobile food dispensing vehicle operator failed to submit a Commissary Notification form to the division for each commissary prior to use. Person in charge is utilizing Rio Restaurant and Lounge for non self sufficient activities such as cooking with no commissary notification charge form submitted. Provided form to operator during inspection.

Mobile food dispensing vehicle license number not permanently affixed on the side of the unit in figures at least 2 inches high and in contrasting colors from the background of the vehicle. License number not on unit at time of inspection. Also, missing decal, person in charge states has received in mail, just has it hung yet.
