Spray bottle containing toxic substance not labeled. Observed unlabeled spray bottle with purple liquid inside at bar.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling vents over kitchen cook line with build up of dust.
Food stored on floor. Observed a case of fries and a case of breaded chicken stored on floor of walk in freezer.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed reach in freezer on cook line next to fryer with build up of ice due to gap in seal on bottom of unit.
Wet wiping cloth not stored in sanitizing solution between uses. Observed wet wiping clothes on counter in kitchen next to 3 compartment sink.

Food-contact surface soiled with food debris, mold-like substance or slime. Residue inside soda gum nozzle and in grooves of soda nozzle. Bartender cleaned nozzle at time of inspection.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Dust over tables 62.
In-use utensil not stored on a clean portion of food preparation or cooking equipment. Knife stored on soiled underside of window pass. Knife removed, washed and relocated.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Grease build up on pipe in door way by fryers. Food debris under window pass where knives are stored. Food debris around door/gasket areas of grill drawers, and bread drawer.
Silver ware in expo area not stored inverted or in a protected manner. Manager had silver ware washed to be stored correctly.
Stored food not covered. Rice in walk-in made Saturday per manager, not covered on Monday. Manager covered product.

Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed mold like substance inside ice machine.
Bowl or other container with no handle used to dispense food. Observed paper bowl with no handle in cooked chicken used for utensil. Manager threw away.
Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed salmon thawed in ROP package without opening. Manager took salmon out of packages.
