Handwash sink used for purposes other than handwashing. Observed maintenance worker starting to rise out paint brushes in hand washing sink. Stopped him immediately. Explained why it couldn't be done. Came with the comment I don't see the difference, it all goes down the same pipe. Operator sent him outside to rinse off as he was going to three compartment sink.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed around chute inside of ice machine some black mold like substance. Observed operator cleaning chute.

Equipment in poor repair. Observed hood suppression system with build up of grease on piping and inside of hood possibly causing system not to function correctly.

Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee sweeping floor, then changing task to tray up food with out washing hands. Observed General Manager coaching employee.
Food placed in soiled container/equipment. Observed condiments squeeze containers with multiple types of sauces stored in a soiled deep pan,and bottle tops soiled with dried on residues from sauces. Observed employee removing container with sauces and washing everything.
Hot water takes too long to reach handwash sink resulting in employees using only cold water to wash their hands. Observed by shake machine, hot water taking more than one minute to get hot. Pressure very low on hot water side.
Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed stacker sauce with temperature of 70F and gel like crust on top of sauce sitting on shelf above food prep line. General manager volunteered to throw out sauce. Coached General Manager on proper procedures.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed no proof of employee reporting. Emailed operator paper work. General manager printed up form, and had employees sign off on while inspection was still going on.
Exterior door has a gap at the threshold that opens to the outside. Back receiving door has a gap on right side of door when you face the door from the inside.
