Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. 1) Employee picked paper of the floor proceeded to grab a container to store Hash brown without washing hands. Employee cashed out customer and proceeded to serve the customer cooked foods without first washing hands. 3) Employee touched personal phone and failed to washed hands.

Per Manager food items in units less than 4 hours. Operator received and put away truck earlier. Items not prepared or portioned today. No Temperature recorded. Observed gasket torn. Manager opened freezer door to quick chill.
