No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. Proof of required state approved employee training not available for some employees. Latonya recently transferred from another store but operator doesn't have access to her training records yet. Printed employee agreement for operator and she had employee complete. Aneisha new employee, employee agreement was signed.
Ceiling/ceiling tile shows damage or is in disrepair. Ceiling tile above large ice machine. Operator stated they have a worked order already submitted.
Floor tiles missing and/or in disrepair. By fryer. Operator states that they already have a work order submitted.
Ice buildup in reach-in freezer and/or walk-in freezer.
Open dumpster lid. Operator closed.
Standing water in floor drain/floor drain draining very slowly. By large ice machine. Operator called in a work order.
Nothing was found. The inspector recorded no violations at all.
