Operator discarded items at will.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed ice machine in kitchen.

Employee touched bare body part and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. At cook line- observed employee using gloves, touch cloths soiled, then prepare a burger for customer; without washing hands or change of gloves. Employee removed gloves and wash hands. Educate operator of proper hand washing procedures.
Certified Food Manager or person in charge lacks knowledge of employee health policy and health requirements. Provide by Email, health policies "Big 6" to operator.
Handwash sink used for purposes other than handwashing. At prep area- observed employee cleaning raw onions at hand washing sink. Educate operator of proper use of hands washing station.
