Per operator, item has been out of temperature for approximately 2 hours. Cooler door was not adequately closed resulting in item falling out of temperature. Door closed properly and freezer door left open to allow rapid cool.
Wet wiping cloth not stored in sanitizing solution between uses. Observed at prep table across from fryer. PIC removed and stored appropriately.
