Handwash sink not accessible for employee use at all times. In dish room a silverware basket and roll of trash bags were in hand washing sink.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed butter (91F - Hot Holding); red sauce (129F - Hot Holding) person in charge started reheating items brought out at 1100.
Required employee training expired for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Two food certifications expired in 2023. Discussed with person in charge.
Spray bottle containing toxic substance not labeled. Observed small spray bottle with clear liquid above prep table unlabeled. Observed spray bottle with bleach water missing label. Person in charge added label.
Food stored on floor. Observed unpeeled onion on floor under food rack. Person in charge picked up onion and placed on unwashed produce shelf.

Observed approximately 50 live flying insects in bar area on floor drains under ice bin and bar tap drains. Person in charge immediately started spraying floor drain killing all flys on floor drain area. Person in charge had employee use bottle brush to clean tap drains and poured quat sanitizer down drain. Observed 4 total flys left in bar area after corrective action taken.
No written procedures available for use of time as a public health control to hold time/temperature control for safety food. Observed whole pizza kept on a 3 hour hold to sell by slice. No written time procedures. Discussed with person in charge and provided documentation.
