Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dish machine 0ppm. Manager primed machine, still 0ppm. Inspector instructed manager to utilize the three-compartment sink to sanitize dishes until the dish machine can be repaired. Inspector instructed manager to sanitize all dishes that were washed today in the three-compartment sink.
Stop Sale issued due to food not being in a wholesome, sound condition. Nine packages of raw tuna thawed inside reduced oxygen packaging on shelf inside walk-in cooler. Tuna is fully thawed and the packaging is not cut or punctured. Package does have a label indicating the tuna must be removed from packaging prior to thawing.

Toxic substance/chemical improperly stored. Aerosol can containing burn cream stored on lower shelf of prep table beside clean pan lids near warewash area. Manager removed the burn cream from shelf during this inspection.

Raw animal food stored over/not properly separated from ready-to-eat food. Two pans of raw tuna stored on wire shelving above several flats of canned sodas inside walk-in cooler. Manager stated the canned sodas were for customers and moved canned sodas to another shelf during this inspection.
Sewage on ground from failed septic system. Septic system backing up onto ground behind building. Operator stated they need a new pump and are already working to fix the issue. Sewage on ground is not in a trafficked area. Inspector reported to the Levy county department of health during this inspection.
Toxic substance/chemical improperly stored. Can of paint remover stored on shelf above Bloody Mary mix in storage closet near bar. Aerosol can of glass cleaner stored on counter beside bottles of steak sauce in upstairs server station. Manager removed all chemicals from areas during this inspection.
Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas. Rough ceiling tiles inside dry food storage area in back of kitchen.
Dead roaches on premises. One dead roach on floor in upstairs server station. One dead roach on floor in closet near restrooms. Manager cleaned up and discarded both during this inspection.

Old labels stuck to food containers after cleaning. Four pans with remnants of old labels stuck to the outside on clean dish rack in back of kitchen. Manager moved all to the warewash area to be cleaned during this inspection.
