Food-contact surface soiled with food debris, mold-like substance or slime: Flat surface around the nozzles of the soda machine dirty with a dark substance. Employee cleaned during the inspection.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly: Tested to 0ppm. Owner/Operator attempted to prime and no sanitizer came up through the hose. Three compartment sink set up for sanitizing the dishes.
Handwash sink not accessible for employee use at all times: Ice scoop and bucket in hand-wash sink in back room next to ice machine. Owner/Operator removed.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin: Ice machine lid interior and shield dirty. Owner/Operator cleaned.
Carbon dioxide/helium tanks not adequately secured: Tank in back storage room not secured to prevent it from falling over. Owner/Operator secured.
A return visit. The problems from the previous inspection had been dealt with.

Rodent activity present as evidenced by 19 rodent droppings found in the following locations: * 1 rodent droppings found on shelf in front of hand-wash sink in back storage room; * 8 rodent droppings found on floor under hand-wash sink next to ice machine in back storage room; * 3 rodent droppings found on floor under ice machine in back storage room; * 7 rodent droppings found on floor under wire shelf to left of reach-in cooler in back storage room. Owner/Operator had employee clean during inspection.
Employee with no hair restraint while engaging in food preparation: - Owner/Operator cooking food without hair restraint. Inspector gave a copy of the Industry flowchart on head and facial hair requirements.
Exterior door has a gap at the threshold that opens to the outside: - exterior door by three door reach-in cooler has a gap at the threshold. Owner/Operator states that it came off the other day and he has a new one to replace it.
