Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Manager picked up dead rodent, went outside to disposed, came back to dishwasher area and failed to wash hands. After instructed, manager washed hands.

Shell eggs not stored at an ambient air temperature of 45 degrees Fahrenheit or less. raw shell eggs (61F - Cold Holding). Per manager eggs on serving cart less than one hour, employee placed back inside walk in cooler, until customer arrives.
