Bowl or other container with no handle used to dispense food. Observed a bowl in their rice. Operator removed it.
Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed the operator take rop frozen fish from the freezer to defrost without removing the fish. Operator removed the fish.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed the owner wearing a smart watch while preparing food. Operator removed it.
Missing drain plug at dumpster. Observed a missing drain plug. Operator stated that WastePro refused to put one on.

Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. See stop sale. Observed 4hr tphc rice not time marked. Operator stated it had been made approximately 1hr ago and time marked it accordingly.
Employee washed hands in a sink other than an approved handwash sink. Observed the operator wash his hands in a prep sink. Instructed the operator on proper hand washing.
Bowl or other container with no handle used to dispense food. Observed two bowls used as scoops in dry food containers. Operator removed them.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed the back ceiling vent with dust build up.

As a repeat violation the importance of following proper protocol with this violation was reinforced with the employee.

Operator unable to provide documentation that aquacultured fish has been raised in a controlled environment and fed formulated feed. Observed no proof of salmon being farm raised or pellet fed. Operator had supplier email him proof of parasite destruction and pelt fed.
