Person in charge states has had open during prep. Items out of temp for approximately 2-3 hours, began-icing down items. Egg wash in kitchen at 70F. Person in charge discarded.
Toxic substance/chemical improperly stored. Hydrogen peroxide stored with food items on shelf above prep table. Person in charge removed. Drano stored on case of dressing in dry storage. Person in charge moved to appropriate location.
Food-contact surface soiled with food debris, mold-like substance or slime. Can opener with debris buildup, smoker shelving and interior with debris buildup, panini press with debris buildup. Cutting boards with black buildup.
Clean-out cap missing from plumbing line. Cap on plumbing line outside back door not permanently attached.
Dead roaches on premises. Observed 6 dead roaches in external dry storage, 4 in glue trap. Also, observed 4 dead flying insects on lids of food product in walk in cooler. Person in charge began removing.
Duct tape used to repair nonfood-contact surface. Duct tape observed to repair ice machine.
Floor soiled/has accumulation of debris. Floor with heavy debris buildup behind fryers at cookline, under dishmachine.
Food stored on floor. Items stored on floor in 1st walk in freezer.
Ice buildup in reach-in freezer and/or walk-in freezer. Walk in freezer to left of walk in cooler with heavy ice buildup.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Mold like substance on fan vent and ceiling in produce walk in cooler. Exterior of panini press with debris buildup.
Utensils in poor condition. One spatula with missing piece in clean dish area. Also observed several in use container lids with cracked edges, missing pieces.
