In the walk in cooler, observed raw white fish (47F - Cold Holding); raw pork (47F - Cold Holding); sliced roast beef (46F - Cold Holding); cooked potato (46F - Cold Holding). Operator stated they checked walk in cooler temperatures at 7am and food was at or below 41F. Operator began to add ice to all time-temperature control for safety foods and opened the walk in freezer to help cool foods. Refrigeration technician is currently fixing walk in cooler.
Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking. Observed cooked potatoes on the cook line without a time marking, operator stated cooked potatoes were removed from walk in cooler at 8:40am. Observed egg whites with no time marking on cook line, egg whites have been out of refrigeration for an hour.

Operator stated hollandaise sauce breaks and are unable to keep it hot. Provided operator with time as a public health control form. Operator began to fill out time control form.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed dish racks heavily soiled with mold like substance.
Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. In the breakfast specials menu, raw smoked salmon and poached eggs are not marked indicating the consumer advisory, operator began marking menu.
