Reach-in cooler interior/shelves have accumulation of soil residues. Observed the interior of the reach in cooler has old dried food debris left on the surface.

Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed splitter added to mop sink, must provide vacuum breaker for the mop sink.
Handwash sink not accessible for employee use at all times. Observed plate stand stored in the front of the hand wash sink located by the walk in cooler.
Drain cover(s) missing. Observed drain cover missing in the mop sink.
