Observed cooked chicken (47F - Cold Holding); shredded cheese (49F - Cold Holding); cooked rice (48F - Cold Holding); cooked potatoes (50F - Cold Holding) above the container fill line in the reach in cooler at the end of the cook line. The operator stated the above food items have been held for approximately 1 hour. The operator placed half of the contents in another container and placed in the bottom half of the reach in cooler.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed the slicer soiled with food debris.
